About this feature
Comply Group has a generic health questionnaire built from reviewing COVID-19 questionnaires provided by states and territories across Australia, consequently creating a questionnaire with broad coverage. Read more about this here.
In Victoria, Australia, the Department of Health and Human Services (DHHS) created a paper-based health questionnaire required to be completed by staff before starting each shift. Download the DHHS paper-based form here for reference.
We have digitised this DHHS questionnaire and provided the ability to turn it on in your Comply Group check-in portal replacing the default generic questionnaire.
Time of shift. This field on the original DHHS form is not automatically added when you enable the DHHS form. For this, use the additional details feature.
Staff only. The DHHS intended this form to be used by staff only. If you also have customers, to implement this you should set up a second location for staff.
Red notices. Worksafe Victoria advises the collection of all submissions (including fails to pass the health questionnaire). To achieve this, enable a Red Notice.
Custom introductory text. Change the "Each staff member should..." introductory text to something more tailored/specific for your intended audience.
Use it at your own discretion. Ensure that before you use this feature it behaves and is worded in a way that is compliant with your industry/state guidelines.
Using this feature
Click the "Edit Location" button
Expand the "Health details" tab
Find the "Enable DHHS staff health questionnaire" section
Turn on the checkbox

Save details to pre-fill the next check-in. People can save their details to automatically fill in the form next time they use a location's QR code or URL.
Welcome message / condition(s) of entry. Add a custom message or a binding condition(s) of entry to the statement at the top of the check-in portal.
